Cooksville’s On-line registration for the 2011-2012 hockey season begins on May 15th.

Again for the 2011-12 season the MHL will be using the Hockey Canada Registration (HCR) System. This system is used by all leagues across Canada and is governed, supported, maintained and developed by Hockey Canada. All members of the GTHL, OMHA, and Alliance will be using this system.
The HCR offers a very secure system which will ensure privacy of information for all members.
All Registration will take place on-line. If you do not have access to the Internet, you can contact your registrar directly or visit the MHL office to become a member and register players. (Please note; some Associations may schedule walk up registration events. Please contact respective Associations for details on those events). All player registrations will require you to:
- Be a resident of Mississauga
- Have a birth certificate number, passport, or citizenship number
- Agree to the City of Mississauga & MHL Fair Play Code of Conduct
Please make sure to follow directions closely when using the new HCR system for the first time. Please pay close attention to the required fields as noted with an asterisk.
Players signed to 2011-12 ”A” or Gold teams do not need to register via the May process as they will have been registered prior, at tryouts.
Please be reminded to have your player’s birth certificate, passport or citizenship number available. Registration requires these valid numbers and will not be processed or approved without them..

House League registration runs from May 15 to August 30, 2011. We cannot guarantee that places will be available after August 30th.

REGISTRATION FEES
Fees for the 2011/2012 season are as follows:
Cooksville “Phantom Cubs” - 4 to 6 yr. olds : $195
Minor Novice to Novice – $350
Minor Atom to Atom – $395
Minor PeeWee to Midget – $435
Juvenile – $225

SIBLING DISCOUNTS (effective after August 7): A $10.00 discount is applied to each of the 2nd, 3rd, 4th, etc. registrant of multi player families.

If you have chosen to pay by Credit Card, there is no need to mail your invoice.
If your player is new to the MHL, you will be required to mail a copy of their Proof of Birth.
Once the registrar has received payment and proof of birth date, you will receive a second e-mail confirming your child’s registration.

Refund Policy
Upon registration in April/May each year, “A” team players will be required to pay a deposit of 50% of the total “A” registration fee with the balance payable not later than August 1st. of that year.
Should any “A” player quit prior to the signing of his or her C.H.A. registration card, $75.00 of the deposit will be non-refundable. Refunds of annual Registration Fees paid will be made only after return of any property and equipment belonging to the Association. An administration fee will be deducted from refunded fees for any registration withdrawn or cancelled, as follows:

No refunds given where a false statement is made on the registration form, e.g. (false address submitted by a NON-RESIDENT of Mississauga).
